Run a visit and sign the SOAP note

Start a visit, record vitals, write SOAP notes with autosave, prescribe, print and sign & lock.

In the appMedical records3 sections

Start a visit

There are three ways in:

  • From an appointment, press Start exam. The visit is linked to the appointment.
  • From a patient record, press New visit.
  • From Medical records, press New visit and choose the patient, vet and chief complaint.

The visit workspace

  1. Allergies & alerts show at the top in red if the patient has any.
  2. Vitals: weight (kg), temperature (°C), heart rate (bpm), respiratory rate (/min) and chief complaint.
  3. SOAP notes: Subjective, Objective, Assessment and Plan. Notes save automatically a couple of seconds after you stop typing; the status shows “Saved” with the time.
  4. AI Scribe: dictate and add the transcript to any SOAP field. See Use AI Scribe.
  5. Prescriptions: press Add and enter the drug name (or pick an inventory product), dose, route, frequency, duration, quantity to dispense and instructions for the owner.
  6. Vaccines given: record any vaccines given today with the next due date.
  7. Files: upload lab results, imaging and photos for this visit.

The sidebar shows the patient's details and previous visits, with a link to the full history.

Finish the visit

  • Print summary prints the visit; Print Rx prints the prescriptions.
  • Create invoice makes a draft invoice for the client, with any prescriptions linked to inventory products already added. If an invoice already exists for the visit, it opens that one instead.
  • Sign & lock completes the record. Notes, vitals and prescriptions are then locked, and the record shows who signed it and when.

Who can sign

Vets, admins and owners can sign visits. If a signed visit needs correcting, an owner or admin can press Reopen. The dashboard's Notes to sign count shows how many visits are still open.
