Consent forms and signatures

Add standard templates, write your own, and have owners sign on screen with a finger or stylus.

In the appForms2 sections

Set up templates

  1. Open Forms. On the Templates tab, press Add standard templates to add four ready-made forms: General treatment & anaesthesia consent, New patient intake, Boarding agreement and Euthanasia consent.
  2. Review each one and edit the wording to match your clinic's policies and local requirements.
  3. To write your own, press New template. Give it a Name, a Type (Consent, Intake, Discharge, Euthanasia, Boarding or Other) and the Form text.

In the form text, these placeholders are filled in when signing: {{client_name}}, {{pet_name}}, {{date}} and {{clinic_name}}. Only Active templates can be chosen when signing.

Have a form signed

  1. Press Sign a form and choose the template (or press Sign beside it).
  2. Choose the Owner and Pet. Read the text through with the owner.
  3. Check the Signer's full name. The owner can draw a signature with a finger, stylus or mouse (optional).
  4. Add notes if needed, such as aftercare wishes or an estimate given.
  5. The signer ticks I agree, then press Sign form.

Signed copies are listed on the Signed forms tab, where you can filter by type, open any form and print it. Deleting a template keeps the signed copies. Only owners and admins can delete signed forms.

Tip

A tablet at the front desk makes signing quick. Vetlyn works in the tablet's browser, so there's nothing to install.
